To work within the integrated locality team to provide specified health and personal care tasks to support admission avoidance and facilitated discharge home. The post will require flexible working. This will include regular evening and weekend shifts, as well as the ability and willingness to travel across the areas of Cambridgeshire Community Services which include Sawston, Linton, Melbourn, Harston and surrounding areas.
